What implants are used in Breast Augmentation surgery?

Artificial implants are used nowadays in breast augmentation. In simple language, an implant can be considered as an artificial bag, made of silicon, filled with either saline or medical grade silicon gel. These implants come in various shapes, sizes and textures. The most commonly used shapes of implants are either round or teardrop/anatomical shape. Saline implants are not commonly used nowadays.

Every implant type and its size have their pros and cons and hence a detailed consultation with your plastic surgeon is necessary for final decision.

What are the different types of breast implants?

Breast implants come in different size, shape, textures and filling materials.

Size: Different size of implants are available in accordance to the volume desired by the patient. They differ in base dimensions and the projection. Even same volume implants come with different projection option. These projection options are called as Profiles. Profiles of an implant come in either small, medium or high variants depending on the projection they offer with the same volume of filling agent inside the implant.

The correct size that is suitable for you depends on your volume expectations, your breast footprint, your native breast volume, your built, your body and chest proportions, the breast skin characteristics well as your plastic surgeons opinion.

Shape: There are two shapes of breast implants.

Any implant has two dimensions.

Consider an implant as a half-cut has ball, with its circular shape at the base and a dome above. The Base being a circle comes in a diameter length measurement, one as horizontal and one as vertical dimension across the chest. While the dome comes in height dimension, i.e. away from the chest. In simple terms the base dimensions are called footprint dimensions and the height is called Projection.

Round implants : These implants have symmetrical footprint dimensions, while the projection is different. They come in various different footprint dimensions and each dimension comes with various projection options. Round implants come with either saline filling or cohesive silicon gel filling.

The biggest advantage of round implants is that even if the implant rotates there won’t be any distortion in the breast shape. Another advantage it gives is the fullness to the upper portion of the breast, which causes the breast to project more away from the chest, also giving out a deeper cleavage.

The disadvantage is that because of the unnatural round shape, the edges of the implants can sometimes be visible as well as felt with fingers from over the skin. Also, because of its even projection in the upper and lower half of the breast, the breast feels like a it has had a ‘Boob Job’ done. With less complications, but on the demerit of unnatural looks, the cost of round implants is less than the teardrop ones.

Teardrop or anatomical shaped implants : These implants can have different or same horizontal and vertical dimensions for the base, but the height dimension is not centered like that in the round implant.

They are usually shaped like a normal breast in upright position. Normal breast have fullness in the lower half and have a gentle slope in the upper half, which share the same anatomical resemblance of a breast. To maintain this shape thus the implants come only with Cohesive Silicon gel fillings.

The biggest advantage of these implants is that they give the best natural looking shape to the breast, which counters the ‘Boob job” effect of the round implants. The disadvantage is that if they rotate they will cause distortion of the breast, unlike the round implants. And so most teardrop implants nowadays come with textured and nano-textured surface, to prevent them from rotating or migrating under the breast. 

 

Surface: The implant surface can be either smooth or textured.

The smooth implants are very soft to the touch. And because of the smoothness, their interaction with the breast tissue is very minimal. So the rate of rotation and migration of the implant in the breast pocket is higher. Also, the rate of capsular contraction is higher in round implants.

The Textured implants, because of their uneven surface, interact with the breast tissue and become immobile. So they don’t rotate, migrate, or have a capsular contraction. However, there is an associated low-grade curable lymph cancer associated with textured implants called the acute large cell lymphoma, the ALCL. It is very rare to occur and is curable.

Filling substance: There are importantly two fillers that are used in breast implants, Saline and silicon gel.

Saline implants are readily available, and are physiological to the body. If the saline implant ruptures, the saline won’t interact with the body and it will get absorbed. The implant can also be filled with any amount of saline in a controlled manner, thus, any asymmetry in the breasts can be managed easily. The disadvantage is that it is water like. And so, there can be ‘splashes’ like ‘splashes of water in a balloon” heard when the breast moves.

Silicon gel implants are basically filled with medically fit Cohesive silicon gel, manufactured especially for breast implants. They give a firm consistency to touch, which feels like a real natural breast. They don’t splash like water in a balloon. The disadvantages are that they come readymade filled, they can’t be refilled externally to change the volume of the implant. So they need to be very carefully selected depending upon their size and the asymmetry of the breasts. Also if the implant ruptures the gel gets collected in the breast pocket and another procedure is needed to remove the implant material.

“Gummy bear implants”, are the latest type of silicon based implants in the market. They are similar to the silicon gel implants, with a major advantage is that it contains form stable silicon gel. That means, even if the implant ruptures, the silicon gel wont spill in the breast pocket surrounding.

What are different breast augmentation with implant procedures?

Breast augmentation procedure is, in simple terms, placing an implant in a pocket created surgically inside of the breast. Now, the procedures vary in terms of either the pocket selection and the incision approach to get to the pocket.

This breast pocket can be either made :

  • Parenchymal: Inside the breast tissue itself.
  • Sub glandular: Underneath the breast but above the pectoral muscle of the chest.
  • Sub pectoral: Under the pectoral muscle of the chest.
  • Dual plane: Half under the muscle and half under the breast.

Pocket selection is dependent on your current breast size, the sagginess, the nipple position and condition of the skin over the breast. Plastic surgeon will. examine it all, measure it all and then will opine about the correct pocket for your implant.

Mostly used are the  , Sub pectoral and the dual plane pocket un todays breast augmentation procedures.

The Sub glandular pocket, preserves the pectoral muscle integrity underneath, as no cut will be given over the muscle. Also thus, by avoiding the muscle, it does not lead to unfavorable muscle force acting on the implant unlike if it was underneath the muscle. Which helps in reducing the animation deformity, displacement of the implant and Double Bubble deformity.

The Sub pectoral pocket, is the pocket made underneath the muscle, and hence it has extra covering of muscle over it as opposed to the only breast tissue covering in the sub glandular pocket. This helps in less detection of the implant on looking or even feeling the breast. This gives a more natural feel to the breast, if a silicon gel implant is used in it. Secondly, it does not disturb the breast tissue anatomy, and so the degree of injury to the blood supply and the sensations of the nipple is decreased than the sub glandular pocket.

The Dual Plane pocket, is a pocket made in such a way as to have benefits of both the above two pockets. In this pocket depending on the breast tissue, its sagginess and the fullness needed, the pectoral muscle is cut accordingly. This allows the implant to sit half under the breast tissue and half under the pectoral muscle.

The type of incision given and the resultant scar after the surgery depends on the pocket decided for the implant to sit in and the type of implant selected. These can be either :

  1. Inframammary approach : Incision is given under the breast in the breast fold
  2. Transaxillary approach : Incision is given in the axilla region and not on the chest
  3. Periareolar approach : Incision is given around the Areola in a cresent shape
  4. Transumbilical approach : Incision given in the umbilical region

Mostly for silicon based implants, and a sub pectoral and dual plane pocket, a inframammary incision is taken. This incision is around 5-6 cm, and is under the breast in the breast fold, and so is inconspicuous. For a saline based implant and a sub glandular or sub pectoral pocket, either periareolar, transaxillary or transumbilical incision is taken. This results in a small 3-4 cm scar, which is well hidden in the axilla, under the nipple or in the umbilicus.

The pocket selection, incision approach selection is based on your breast condition and size, the size of implant needed, the type of implant selected and the measurements and opinion given by the plastic surgeon. Always consult in details with your plastic surgeon regarding the same.

What preparation is needed before the surgery?

Once you are done with your thorough consultation and general examination for breast augmentation with your breast surgeon, unit two undergo the next steps

Fat injection may or may not be performed along with the procedure depending upon the amount of fullness needed, the quality of breast tissue over the implant and the final touches for smoothness of the curves.

  • Anesthesia: The first step is giving you general anesthesia by anesthetist, it is usually a safe and smooth process of getting you under anesthesia, our expert anesthetists make sure you have a smooth surgery that you wont recall once you wake up.
  • Incision: There are multiple incision options that can be used by the plastic surgeon depending upon the type of breast reduction being performed
      1. Inframammary Incision : Incision is given under the breast in the breast fold
      2. Transaxillary Incision : Incision is given in the axilla region and not on the chest.
      3. Periareolar Incision : Incision is given around the Areola in a creasent shape
      4. Transumbilical Incision : Incision given in the umbilical region

Pocket creation : Depending on the breast pocket selected before the surgery, a pocket is created by the use of cautery in a blood less plane either in

  1. Sub glandular: Under the breast tissue.
  2. Sub pectoral: Under the pectoral muscle.
  3. Dual plane: Half under the pectoral muscle and half under the breast tissue.
    • Suturing : Once the breast implant is positioned properly, fixation sutures are taken inside the breast tissue.  A new inframammary fold is created to accommodate the increase in volume of the breast in order to hide the scar as well as to give a natural look to the breast. The skin is draped over the breast tissue and  suturing is done from within, so as there are no external sutures. Skin glue and surgical tapes are used to enforce the outer skin. Care is taken as to properly suture the incision, as to have practically no scar.
    • After surgery : Once suturing and dressings are done, a supportive bra is placed on the table itself before weaning of the anaesthesia. This avoids the discomfort and helps in smooth recovery from anaesthesia after the surgery. Once you are awake you are shifted to the recovery room for further monitoring. 4 to 6 hours after the procedure you are allowed to have normal diet.

When should I expect the results of my breast augmentation?

You will notice immediate results after breast augmentation surgery. The tissues and the scars require time to heal properly and so it will take a few weeks for complete healing and scar reduction after breast reduction surgery

What is the after care needed after breast augmentation?

Breast augmentation surgery usually is a day care procedure. You can be discharged in the evening of the surgery once you are comfortable to move around.

There is no limitation for bed rest after a breast augmentation surgery, we advise early mobilisation of patients out of the bed as soon as the patient is comfortable.

A support bra will be asked to be used for 4 to 6 weeks after breast augmentation to give proper support for healing of the breast. 

You usually can go to work after 4 to 5 days of surgery depending upon your comfort level. Strenuous activities, exercises, jumping are usually prohibited for a certain period after surgery. there are no external sutures used so there is no need of any suture removal.
